As recognized components of AAPA, constituent organizations are entitled to specific benefits and resources. To maintain an organization's charter or recognition, routine reports must be submitted as well.

Below are links to on-line forms for various resources and services.

Constituent Organization Update and Re-chartering Forms

  • Constituent Organization Officers Update Form: This form is used to update the constituent organizations officers. This should be filled out twice a year, in January and July. The information can be updated any time there are changes in officers.
  • House of Delegates Notification Form: This form is used by chapters, specialty organizations and the caucus congress to designate their delegates and alternates for the AAPA House of Delegates.

Starting a Constituent Organization or Special Interest Group (SIG)

  • AAPA Chapter Application (coming soon): This is a form to be used by an organization wishing to be a chartered chapter of AAPA.
  • AAPA Specialty Organization Application: This is the form to be used by an organization wishing to be an AAPA-recognized specialty organization.
  • AAPA Caucus Application: This is the form to be used by an organization wishing to obtain caucus status with AAPA.
  • AAPA Specialty Interest Group Application: This is the form to be used by an organization wishing to obtain special interest group status with AAPA.

Constituent Organization Development and Support

  • Mailing Label Request Form: AAPA will provide postal service addresses for selected constituents. The addresses may be sent to the constituent organization in an electronic file. These addresses are for one-time use and may not be sold or traded to any outside parties.
  • E-mail Communications Form: AAPA will send an e-mail blast to selected constituents using text provided by the constituent organization. The e-mail will appear to be delivered from the constituent organization to the recipient. Text for the e-mail communication must be received electronically either through the on-line form or by e-mail. A prearranged fee will be charged to the constituent organization before the order can be processed.
  • CME Announcement Form: This form is for AAPA-approved constituent organizations only to submit an announcement for their CME programs.
  • Directors and Officers Insurance Form: This form is for enrollment in the AAPA sponsored insurance program. Directors and Officers Insurance (D&O) should be maintained by all constituent organizations to protect your board members' personal assets in the event of a lawsuit.
  • General Liability Insurance Form: This form is for enrollment in the AAPA sponsored insurance program. General Liability Insurance (GL) should be maintained by all constituent organizations that host on-site events to further protect your organizational assets in the event of a lawsuit.
